As Private Adatıp Hospital, we consider protecting patient safety, reducing risks, and achieving continuous improvement through learning from incidents at every stage of healthcare delivery to be fundamental elements of our institutional culture. We also address the safety of patients’ relatives, visitors, and employees within the relevant processes.
In line with these principles, we commit to the following:
1. Compliance with Legislation and Standards: We comply with applicable patient safety legislation, quality and accreditation standards, and institutional procedures.
2. Risk Management: We identify and assess risks associated with diagnosis, treatment, care, and support services and implement preventive measures appropriate to the level of risk.
3. Patient Identification and Safe Transfer: We ensure that the correct procedure is performed on the correct patient and safely manage patient admission, handovers of care, transfers, and discharge processes.
4. Medication Safety: We implement necessary controls throughout the procurement, prescribing, preparation, storage, distribution, administration, and disposal of medications and monitor adverse drug reactions.
5. Surgical and Interventional Procedure Safety: We implement patient, procedure, and surgical site verification processes, surgical instrument and material counts, and anesthesia and sedation safety practices.
6. Safe Patient Care: We assess the risks of falls, pressure injuries, and care-related harm and implement preventive measures appropriate to each patient's condition.
7. Infection Prevention: We ensure the effective implementation of hand hygiene, aseptic and antiseptic practices, isolation precautions, cleaning, disinfection, and sterilization procedures.
8. Blood Transfusion Safety: We perform the necessary controls for ordering, preparing, storing, distributing, and administering blood and blood components and evaluate transfusion reactions within the scope of hemovigilance.

We do not use incident reports for punitive purposes or individual performance evaluations. We protect confidentiality and employees who report safety concerns in good faith.
16. Promotion of Safe Practices and a Just Culture: We encourage employees to raise concerns about unsafe practices without fear of pressure or retaliation. We evaluate incidents objectively, considering both human and system factors without prejudice.
17. Incident Analysis: We investigate incidents according to their type, whether they reached the patient, the severity of harm, and the level of risk. We conduct root cause analyses for serious unexpected events and apply the Plan-Do-Check-Act (PDCA) cycle, Corrective and Preventive Actions (CAPA), and Failure Mode and Effects Analysis (FMEA) when necessary.
18. Improvement and Committee Monitoring: We assign responsible personnel and establish deadlines for improvement activities. We monitor their completion, effectiveness, and sustainability and report the findings and evaluations of the Patient Safety Committee to hospital management.

20. Monitoring, Training, and Management Responsibility: We monitor patient safety outcomes through workplace assessments, risk analyses, performance indicators, incident reports, and safety culture assessments. We provide employee training, allocate necessary resources, and review this policy at least once a year and whenever necessary.
